The SOHL team will guide your organization towards a primarily wellness-focused model of care, identify opportunities for value-based care negotiations, and create key metrics to calculate the ROI of wellness initiatives.
How It’s Conducted:
- SOHL project team interviews health system team members in key areas, interviews are both individual and system-focused. Willing interviewees take the SOHL self-care assessment.
- SOHL project team interviews a sampling of system-wide stakeholders [patients, providers at all levels and specialties, employees, board members, leaders, individuals in partnering organizations, and other relevant stakeholders].
- SOHL team reviews relevant documentation, processes ICD-10 codes from billing data through the SSC algorithm to identify optimal benefits and interventions for the patient population, invites all interviewees to attend an SSC training session.
- SOHL team processes and analyzes information from all of the above steps, create a draft report with executive summary presented to the health system team leads. SOHL team obtains feedback prior to creating a final report and executive summary.
- SOHL presents reports, recommendations, preliminary SSC initiative implementation, and ROI projections.
The Outcome:
- Improved understanding of the system’s strengths in supporting and enhancing wellness as well as areas for improvement.
- A preliminary plan to implement a Sustained Self-Care Initiative (SSCI) to evolve to the wellness model of care.
- Enhanced understanding of current wellness levels for all stakeholders based on assessments, interviews, and training.
